Account Executives: Willow Services is seeking experienced account executives for Kane and Will counties for our home health, hospice, and private duty services. Account executives should embrace our WE CARE values as they meet with patients, families and medical administrators and professionals to present our Circle of Care, and our impressive attributes, including our accreditation by the Joint Commission, the HomeCare Elite award for the past two years and consistent high quality ratings from CMS, including a recent 4.5 out of 5 stars. On-Call RN: We are looking for a Weekend On-Call Home Care Nurse. We prefer at least one year of home care experience. Duties: Take calls for the agency, process referrals and other communications that come through our efax, communicate with Clinicians, families, docotors, and patients as needed. Must be willing in learning our Kinnser software system. Must be willing to see patients for obtaining blood draws/specimens, performing wound care, IV, foley, and catheter care if necessary as a last resort if coverage needed.

Nurse Auditor: We are looking for a highly organized, self-motivated, and detail-oriented registered nurse to perform chart audits. This nurse will review, code, and score OASIS assessments, track results by individual, conference and support clinicians as appropriate to address charting deficiencies, function as a resource person to field clinicians regarding documentation, and assist Director of P.I. and Education in job related activities as needed. This nurse will be trained to be knowledgeable regarding agency policies, procedures, and accreditation requirements, including state and federal regulations. Qualifications: Registered Nurse, able to function autonomously and to work well with others. Preferred: At least 1 year of experience in home health, proficiency in OASIS and coding.

Registered Nurse (RN): Willow Home Care, LTD is looking for experienced Registered Nurses to join our Home Health Care team. We believe that home health care is an essential part of the recovery process and are committed to providing patients with quality healthcare professionals that will meet their individual and unique needs. The Home Health Care Nurse is a fundamental team member providing quality-nursing services to patients in the comfort of their own home. Physical Therapist (PT): We are seeking experienced Physical Therapists (PT). The physical therapist utilizes physiological and psychological knowledge to care for a variety of patients and their significant others. PT's must exhibit the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. This entails that the individual show knowledge of the principles of growth and development as well as the physical, emotional and psychosocial needs of the patient population served. PT's work closely with other members of the health care team. We are looking for staff that understands the importance of a changing industry and the emphasis on efficient, effective, safe care in a timely manner.

Occupational Therapist (OT): Willow Home Care, LTD is looking for experienced Occupational Therapists (OT). OT's provide assessments and treatments of patients requiring occupational therapy services within the patients home. Provide professional occupational therapy visits of patients and their families in accordance with physician's orders through direct care, supervision and multidisciplinary team coordination. OT's are a resource to the Field Staff in the areas of community rehabilitation and individualized rehabilitation needs of the patient.

Speech Therapist (ST): We are seeking Speech Therapists (ST) with Medicare home health experience. Requirements include providing quality care and coordination of care in conjunction with patient's needs, their families, physicians, nurses and other clinical staff, and payers.

Certified Nurses Assistant (CNA): Willow Home Care, Ltd is seeking compassionate Certified Nursing Assistants (CNA) to work within our clients' homes offering support with daily living tasks, and personal care under the supervision of a Registered Nurse. The qualified Certified Nursing Assistants (CNA) works together with patients and are responsible for basic care services like bathing, grooming, and eating. Willow CNAs ensure our patients receive essential social and emotional support and provide vital information on patient conditions to nurses.
